On 9/20 and 9/30 I completed my third and forth shift on 7 West at Sharp Memorial Hospital. Although these were not my first shift on the unit they were my first shift with my regular preceptor Elle. The first two shifts I was working with Laura who was my temporary preceptor, while Elle was on vacation. The first night was calm because of low census and the second night it was a bit busier throughout the night. On our first shift together we were the resources nurses, which was an interesting and new role for me to take on. During this shift I believe the overall theme that shined through was humility. This shift I worked on my communication at hand off report by providing the shift to shift RN hand off report on one of my patients. Before giving hand off I mentally practiced it through a couple times. Then I practiced it by pretending Elle was the nurse that I was giving hand off report to. Finally I was gave report for real to the day nurse and it went really well. I think the that with more and more practice I will gain the confidence I need to be comfortable giving report without practice. My goal competence with this task is an 8 and now I feel my competnece level is a 6. In regards to telemonitoring I gained experience by first analyzing my patients strips and then gaining more experience and practice by going online and looking up practice testing. A lot of the time we have down time during the hours of 2 am to 5 am and this allows us to sit down and practice reading telemonitoring strips. My goal competence with this analyzing telemonitoring is a 5 and my new competence level since having this practice time is a 3. This shift I did not get to practice many new skills, but I did learn a lot about the importance of building a trusting and caring relationship with your patients. A lot of the tasks I completed were not glamorous. Wiping someones dirty bottom, performing foley care, getting the patient a blanket, or helping a patient ambulate to the restroom are not the most desirable jobs. This shift taught me that although these actions may not seem too important to us, but to the patient these actions make all the difference.
