Clinical Context and Experience
On 9/20 and 9/30 I completed my third and forth shift on 7 West at Sharp Memorial Hospital. Although these were not my first shift on the unit they were my first shift with my regular preceptor Elle. The first two shifts I was working with Laura who was my temporary preceptor, while Elle was on vacation. The first night was calm because of low census and the second night it was a bit busier throughout the night. On our first shift together we were the resources nurses, which was an interesting and new role for me to take on. During this shift I believe the overall theme that shined through was humility. As I worked as resource nurse I worked with an eclectic mix of patients that I normally was not aware of their

This shift I worked on my communication at hand off report by providing the shift to shift RN hand off report on one of my patients. Before giving hand off I mentally practiced it through a couple times. Then I practiced it by pretending Elle was the nurse that I was giving hand off report to. Finally I was gave report for real to the day nurse and it went really well. I think the that with more and more practice I will gain the confidence I need to be comfortable giving report without practice. My goal competence with this task is an 8 and now I feel my competnece level is a 6. In regards to telemonitoring I gained experience by first analyzing my patients strips and then gaining more experience and practice by going online and looking up practice testing. A lot of the time we have down time during the hours of 2 am to 5 am and this allows us to sit down and practice reading telemonitoring strips. My goal competence with this analyzing telemonitoring is a 5 and my new competence level since having this practice time is a 3. This shift I did not get to practice many new skills, but I did learn a lot about the importance of building a trusting and caring relationship with your patients. A lot of the tasks I completed were not glamorous. Wiping someones dirty bottom, performing foley care, getting the patient a blanket, or helping a patient ambulate to the restroom are not the most desirable jobs. This shift taught me that although these actions may not seem too important to us, but to the patient these actions make all the difference. This week at clinical I had a Gyn patient for two days and a new mom and baby on the second clinical day. My patient that had on Tuesday with the hysterectomy was a very quiet and kind lady who was recovering very well from her surgery. During and after her cares we were able to connect through learning about her family she grew up with, her family she had grown of her own and about her children and other interests of hers. It made me feel good being able to own the moment through connecting with my patient that also made collaborating and working as a team much easier throughout the day to complete the needed tasks. Another aspect of making the shift go very smoothly was working with Brenda, I enjoyed collaborating with her as she is fast-paced…

I've learned plenty throughout the past eight weeks. Looking back, the nursing process and cultural competence were the two concepts that I took the most away from. Learning the nursing process has taught me the importance of providing patient-centered care. Since then I have began to apply the steps of the nursing process while working as a nursing assistant and it has changed the quality of the care that I provide. I plan to bring what I've learned about the role and responsibilities of a nurse to my future clinical rotations.…

Since researching this topic, my nursing practice has changed in the way that I interview, assess, and treat my clients. Throughout my education in nursing school I was always taught a more holistic approach, however I was never truly conducting my assessments and interviews with clients within this matter. I am always respectable and caring towards clients, but in my past practices within clinical, my focus was primarily on the disease process and preventing the disease from worsening. Within this semester I have improved my practice, with the help of this research, to not only treat the disease process, but to use the same focus as CAM and treat the person as a whole. I found this to also help in establishing better client-caregiver relationships.…

Clinical started with all of us meeting in the main lobby of Methodist Northlake Hospital. I was a bit nervous for I was going to be a student nurse manager for people I never met before. I didn’t know if they would take me seriously. I took a long breathe in and slow exhale out, before I introduced myself to the clinical group. We then headed to the Labor and Delivery floor to change out of our clothing and into hospital scrubs.…

Thus far, I have learned many important aspects of becoming an effective therapist. One of the major parts of training involves evaluation and feedback from supervisors or peers. I think evaluations are an important part of training because it provides feedback to areas, which you are doing well in, and areas in which you will need improvement. I believe that without evaluations, we as therapist cannot grow into effective therapist. To date I cannot think of any job or career that does not use “end of the year evaluations”.…

Within the local hospital, continual complaints of non-comprehensive and inadequate end-of-shift reports stands out as a chief concern. With the concern of inadequate handovers, the concern of poor communication among nurses in the medical-surgical department and a lack of teamwork also are noted. A voiced concern among nurses about the amount of errors rooted in poor communication within the facility also sparks interest into a change to improve communication processes. Therefore, the facility recognizes a need for change and willingness to attempt to improve the patient handover process. Additionally, a large number of new graduate nurses and new hires have occurred…

RN III to Clinical Nurse Resource Promotion I graduated from nursing school in 2011 and accepted my first nursing position here at Wheaton Franciscan Healthcare Saint Joseph Hospital in the Emergency Department that same year. I started my orientation with two other staff nurses and began working independently after eight weeks. I remember that I felt so unsure of myself and was frequently asking others for their input on patient cares. After I had been here for about one year, I recall I found this new confidence in my ability to care for my patients. I had begun to grow an encompassing knowledge of involving the different disciplines and the vast contributions a multidisciplinary approach has on patient care.…
