When talking about multiple case managers managing a case, it involves a lot of communication skills. It brings you back to the true meaning of multi-disciplinary team concept. This involves the major elements to coordination: managing the tasks regarding the patient pathway during a period of care, organization of the multidisciplinary team and responsibilities involving implementation of the care plan as far as intervention for the client. In theory the case manager is the catalyst for the team due to the various players involve with the care of the client.
