This aims to provide a handy self-instructional for SWs, Center Heads and staff and other readers’ to understand the strategies and how these are used in case management. The interventions are applied starting from case admission, intake assessment, during recovery and reintegration to realize the goals of successful reintegration of rehabilitated CNSP cases. Both the MSSM and TBCM are tools for assessment and timely case management while the TransFER and SIGLA are intended for cases needing either transition homes or independent living arrangements.
