Ms. Spivey is a 23 year old female who presented to the ED via EMS after overdosing on an unknown amount of un-prescribed medications. At the time of the assessment Ms. Spivey reports only remember taking one pill for her back pain and waking up in the ED. She denies attempting to harm herself, homicidal ideation, and symptoms of psychosis. Ms. Spivey reports a history of depression and anxiety. She expresses relational issues, financial issues, and unemployment are stressors contributing to her distress. Ms. Spivey reported depressive symptoms as: feelings of worthlessness, hopelessness, isolation, irritability, tearfulness, and despondency. Ms. Spivey reports a history experiencing domestic violence for her relationships. She reported tonight spending time with some friends and taking multiple pills with intent to "get high". During the assessment Ms. Spivey appear malingering about the amount of medications taken by evidence of admitting she has been untruthful about initial statements. Spivey mother and sister was contacted by QP. The mother reported her daughter has a history of hospitalizations for mental at the age of 14 for overdosing. Ms. Spivey admits to being hospitalized around that age, however she reports it was because of her behavior and marijuana use. Ms. Spivey sister reported to nursing Ms. Spivey reported to her she was suicidal and wanted to die. The sister was confronted about claims, Which she reports her sister didn't report that information to her at any point during this incident. The sister reports Ms. Spivey has informed their family on several occasions she did not want to be here (referring to living in her current situation) and wishing it could be over.
