Ms. Newby is a 30 year old female who presented to the ED via LEO under a custody order. Dr. Ryan Snyder requested an evaluation on Ms. Newby to determine disposition. Per documentation Ms. Newby threaten to stab her mother today with a knife. per documentation Ms. Newby has been seeing and hearing people around her home when no one is there. reported to ED staff she has a history of mental illness and has been treated for the same. At the time of the assessment Ms. Newby adamantly denies suicidal ideation, homicidal ideation, and symptoms of psychosis. She reports today around 1:30pm her mother and her got into an argument over her mother's Facebook password code being changed. Ms. Newby reports she is visiting her mother from Florida. Per documentation she reports taking adderall years ago and has a history of ADHD. Ms. Newby denies feelings of depression. She states, "I'm positive all the time, I don't like to be negative." Ms. Newby expresses she is upset about the situation and would just like to get through it. This clinician attempted to contact Ms. Newby mother 3 times tonight. 2 voicemails were left to contact Randolph hospital or TACT as soon as possible to collect collateral information. …show more content…
Newby most recent UDS she tested positive for Methamphetamines and Amphetamines. Ms. Newby reported only using marijuana 2 weeks ago. Ms. Newby reports she started smoking marijuana at the age of 18 and usually use marijuana daily. When confronted about UDS results she appears guarded and become tearful. She reported before being asked about results not knowing the last time she took and medication. She states, "I don't never remember when the last time I took anything, the doctor said she could prescribe me anything because I lived in Florida and i am in North Carolina." She expressed if she did take anything it was months
