Ms. Corson Case Study

D/A: There were no medical appointments for Ms. Corson this week. The consumer met her psychiatric Dr. Capiro on 01/23/17 and was provided a direct service at the Residential Treatment Program. The service included a comprehensive psychiatrist evaluation, medication review and an assessment of her needs. The review and assessment determined that medications were appropriate and consistent with her current health condition. Ms. Corson continues to be compliant with her medication and treatment and continues to follow the rules of the Residential Treatment program.
On 01/24/17, staff assisted consumer with her laundry. Consumer reported to staff on their way back from the Senior Center that she dislikes the new van. When his Case Manager asked Ms. Corson why? She stated “I just don’t like the van.” Consumer and Mr. X went for a ride with Case Manager on 01/28/17, into the community for 45minutes. Staff continues to praise Ms. Corson for completing her ADL’s independently and helping out with house chores (sweeping, mobbing etc.)

Corson met her treatment goals this week by going to the Frederica Senior Center three times this week on 01/24/17, 01/25/17 and on 01/27/17. The consumer told staff she had fun and looking forward to the following day. Ms. Corson met her goal this week to become healthy this week by eating food low in sodium and fat (chicken & rice).
P: Staff will continue to encourage and support Ms. Corson to reach her
