Mr. Ware is a 17 year old male who presented to the ED via LEO with suicidal ideation with multiple plans. At the time of the assessment Mr. Ware currently denies suicidal ideation, homicidal ideation, and symptoms of psychosis. He reports tonight his mother and he got into an argument over cleaning his bed room. He became angry and decided to walk to calm down. He expressed his mother would not leave him alone, so out of anger he stated he would harm himself by drinking bleach, overdosing on pills, or stick a fork in a light socket. Mr. Ware sated to his mother, "I told her i was angry and could she please leave me alone." Mr. Ware reports his mother's abusive relationship with his stepfather as the primary stressor contributing to his distress. He reports feeling worthlessness and angry due to his distress. Mr. Ware does not appear to be exhibiting signs of agitation, aggression, or responding to internal stimuli. …show more content…
She reports while her son was taking a shower she went into his room and saw that he need to clean it up. The mother reports explains an argument occurred, then he left, walking down the side of the highway where the stay in the dark. She reports she feared for his safety and called the police. The mother reports her son has been diagnosed with ADHD in the past, has a history of cutting. He admits to cutting himself 2 weeks ago with a pencil. He expresses it was not to harm himself, but he became angry with the conflict in the home. The cuts appear superficial. The mother reports her son is not on any medications, has not seen an outpatient provider since being diagnosed with ADHD. She reports no hospitalizations for mental health as well. Mother was able to contract for safety with her son. She admits to being in a abusive relationship and currently attempting to get out of it. Hospital staff provided her with referrals to family
