Mr. Spivey is a twenty-nine year old White American male. He stated that he was born in Charlotte, North Carolina. He reported that he lived in Atlanta, Georgia consistently for fourteen to fifteen months. Mr. Spivey reported that he is single and does not have any children. Mr. Spivey has been required to complete a substance abuse evaluation as a result of a possession of an illegal drug related offense. The evaluation is to determine whether Mr. Spivey is suffering from a Substance Related Disorder and to recommend a course of treatment, if appropriate. The interview was conducted directly and entirely in English, Mr. Spivey’s native language. II. EMPLOYMENT/EDUCATION HISTORY

Mr. Spivey reported that he currently works at Merritt Hawkins, which is located in Roswell, Georgia. He stated that he has a position in sales. Spivey reported that he was eighteen when he consumed Adderall while he was in his dormitory. He stated that the Resident Assistant gave him Adderall while he was in undergraduate school. Mr. Spivey stated that he consumed Adderall once a week. He stated that he consumed 10 mg of Adderall when he first began to consume the drug. Mr. Spivey stated that he began to consume 20 mg of Adderall over time. He stated that he began to consume again six months after law school and increased his dosage to 60 mg. He eventually received another prescription to take Adderall. Mr. Spivey reported that it took twenty minutes to begin feel “awesome, energetic, focused, a certain calm, smart, certain, and capable” within two hours of consuming Adderall. Mr. Spivey reported that he would not always have a prescription by a doctor to use the Adderall. He stated that he would get the drug from other people.
