Mr. Miller Case Study

Since 5/3/18 I have spoken with Mr. Miller. He reports he was hesitant to speak with me that he had a nurse case manager from his attorney. Even after the adjuster explained that I was related to the work portion he would not speak with me. I made many calls as requested by the adjuster without reaching Mr. Miller. The MRI and soft collar had been approved and I was unable to make those arrangements without speaking with Mr. Miller. Mr. Miller’s attorney finally contacted me and we agreed to meet to discuss my role and Mr. Miller's responsibility.

On 5/11/18 I met Mr. Miller at the office of Attorney David Alexander. Mr. Alexander is representing Mr. Miller for the auto portion of his claim. He is not representing him for the work comp

Miller verbalized understanding. He read and willingly signed the Authorization for Release of Medical Information form. Mr. Miller’s attorney Mr. Alexander read the consent and approved his signing.

ATTORNEY

Mr. David Alexander, 4800 Fashion Square Blvd, Saginaw, MI 989-799-0101. He is representing Mr. Miller on the auto case, not the work comp.

IMPRESSIONS

Mr. Miller has a history of hard lumbar cutting employment. He has had a fall with a compression fracture at T8. He has had Kyphoplasty in 2013. Mr. Miller has been on consistently high doses of Morphine even 5 years after the Kyphoplasty. He smokes, his age, he is overweight, his constant usage of high levels of opioids, and he is not overly physically active, these issues will impair healing and recovery. The Official Disability Guidelines for a cervical strain or Whiplash is 25 to 34 days to recovery. PLAN/ RECOMMENDATIONS

1. Contact Mr. Miller every 2 to 4 weeks. Obtain an update on his current pain levels and physical symptoms.
2. Attend the next appointment with Dr. Bodrie when made.
3. Attend the appointment with Dr. Mark Adams when made. Obtain a treatment plan and projections for recovery.
4. Facilitate the MRI and medical care as directed by the treating
