Mr. Chavis Case Study

Mr. Chavis is a 24 year old male who presented to the ED after being found wondering around a closed business in his boxers. IT was determine he was under the influence of some unknown substance. Before the assessment Mr. Chavis grandmother was spoken to. She was with him upon arrival to the Hospital. She reports relational conflict late yesterday afternoon triggered her grandson "getting high". She states, she does not believe he was attempting to harm himself. Per documentation Mr. Chavis denies suicidal ideation, homicidal ideation, and symptoms of psychosis. At the time of the assessment Mr. Chavis is found just waking up from an hour nap. He is calm and cooperative. He reports taking 5-6 Wellbutrin along with other prescribed medications
