Ms. Pauley is a 16 year old female who presented to the ED following sending her mother text messages in regards to ending her life. Ms. Pauley reported to her mother she took an unknown amount of Motrin soon after. She reported engaging in overdose due to relational issues with her step-father. Dr. Keith requested an assessment on Ms. Pauley. At the time of the assessment Ms. Pauley denies suicidal ideation, homicidal ideation, and symptoms of psychosis. She appears angry and guarded at the beginning of the assessment. Ms. Pauley does admit to sending the text messages to her mother tonight because of conflict with her step-father. She reports researching about the medication before she took medication. Ms. Pauley expressed depressive symptoms. She symptoms include: feelings of worthlessness, anger, irritability, tearfulness, and isolation. Patient …show more content…
Pauley mother, who was at her bedside states her daughter locked herself in her room and she had to force herself in. The mother states her daughter informed her of taking an unknown amount of a new bottle of Motrin. The mother reports counting the pills and found 11 missing. This clinician was shown the text messages Ms. Pauley sent to her mother. The text messages stated (6:27pm 4/2/17), "Just sign me over to someone I'm tired of putting up with Greg and being put down if you don't sign me over this time I'ma just take my life. She also stated (6:47pm 4/2/17), "...your happy have a great life with him. And just like I said I will kill myself before any of that happens 100 I hope he cheats on you."

Due to an increase in depressive symptoms, suicidal ideation with a plan, recent attempt, potential harm to self patient does meet criteria for IVC and inpatient hospitalization. TACT consulted with Dr. Keith, it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate placement.
