Mr. Thomas is a 16 year old white male who mother, Teresa Snyder, referred him to Mobile Crisis Management (MCM) at 2:09pm in regards to her son's recent behavior. She reported on today's date her son has been defiant, grabbed her arm, and destroying property. Dispatcher contacted Qualified Professional (QP) to respond to crisis. Before arrival QP contacted Sandhills Center at 2:41 pm and spoke with licensed professional, Tonya. Tonya reported Mr. Thomas attended Daymark Recovery Services, at 110 W. Walker Ave Asheboro, NC 27203 in 2015, and was hospitalized at Moses Cone Memorial Hospital in Greensboro, NC in 2015.Tonya reported Mr. Thomas has a history of ADHD, ODD, and depression. QP contacted Mrs. Snyder who wanted to meet at 962 S. Fayetteville st. Asheboro, NC 27203 due to expressing her residence was not appropriate to dispatcher. QP informed Mrs. Snyder of estimated arrival time. QP arrived at location at 3:26pm. …show more content…
Mr. Thomas reported experiencing moderate anxiety. He also appears guarded. He denies suicidal ideation, homicidal ideation, and symptoms of psychosis. Mr. Thomas reported last month his step-father ended the relationship with his mother and moved to Kentucky. Mr. Thomas since this his step-father moved his mother and he has been experiencing worsening relational issues. Mr. Thomas reported his mother has been consuming alcohol more frequently. Mr. Thomas reported his mother and he have been experiencing physical abuse by his step-father wile he was living with them. He reported a few weeks ago his mother was drinking and threw a empty glass bottle of Alcohol at him, which hit him on the ear and caused his ear to bleed.
