Needs Assessment Purpose: Through the needs assessment we will determine how we can increase the physical activity of WCSU students through the understanding of health needs as a campus community by use of the WCSU Recreation facilities.
Focus Group Purpose: Through our focus group we will determine the best possible way conduct our project by identifying the mission and values of the WCSU Recreation Department, what programs are currently offered, the best way to communicate with/ to WCSU students, what HPX 371 students wish they had done differently to ensure success with our HPS service learning project, and what

With that being said, it is only for this focus group. In our project your names will not be attached to any of the comments or answers given. We ensure you all of complete confidentiality to the fullest of our capability.

Address Audio Recording: This focus group will be recorded today using an audio recorder to help assist us with the accuracy of your responses. The audio recorder is just in place to make sure we don’t miss any of your comments being that we can’t always right then down fast enough throughout the discussion. You all have a right to refuse to the audio recording. After the completion of our project all recordings will be erased.
Address Incentive and Voluntary Participation: Everyone’s participation here is voluntary; you can withdrawal from the discussion at anytime. All the information that is gathered from your comments, discussions, and answers will be analyzed and used to help us implement a program that will benefit WCSU students through the use of WCSU recreation facilities. The creation of our program is nearly impossible without your help and cooperation. There are few to none minimal risks that will be associated in our discussion today. Taking part in this focus group is a verbal agreement and consent to go forth and participate.

The information collected will then go into a report that will be reviewed and submitted by our professors in HPX 200. We will then use the information to determine where our priorities should be when it is time to implement a program for WCSU students.
Individual Introductions: Before we begin I would like to draw to everyone’s attention that the name cards sitting in front of everyone are there to help people remember each other’s names. I would also like if we could all go around and introduce ourselves, starting from my right.
Focus Group Questions: Does everyone understand the purpose and process of this focus group? Please ask any questions that you may have before we begin. The audio recorder is going to be turned on now. Let’s begin.
1) Take a moment to think about primary health concerns on campus. How would increasing physical activity of WCSU students decrease those primary health concerns? Why?
2) What resources are available to help develop/ implement new health programs? What is the best way to get information about the program to WCSU
