R/s on 10/10/2017, Thomas enrolled Carole into school and days later he requested homebound forms. R/s Thomas said he was trying to get Carole Ann an appointment at Waccamaw Mental Health, but her intake appointment is on 11/15/2017. R/s today Carole Ann has a 1:30 pm appointment at Little River Medical Center to get the homebound forms filled out.
ALLEGATIONS:
R/s the family moved to South Carolina from New York in September. R/s Carole Ann (15) should’ve started school on 09/12/2017, but she has yet to attend school. R/s Carole Ann’s school in New York suggested Carole Ann to be on homebound and she had an IEP in New York. R/s Carole Ann has severe anxiety, ODD, and mood disorder unspecified, and ADHD.
