Your patient Mirza Barlas was referred to the Region II Putting on AIRS Program upon his release from CCMC on November 17th. Enclosed is his signed release of information permitting us to send the summary of his home visit on 12/30/15. In addition, to his completed Asthma Control Test (ACT). This test is taken from the Measures of Asthma Assessment and monitoring document page 67; “Patient Self-Assessment” (EPR-2 1997). Which, Mirza completed himself at the visit.

During the visit, we reviewed in detail using model lungs, illustrations, literature, and an asthma video; asthma pathophysiology, the purposes, preferred delivery methods and care of all his asthma medications; including their means of delivery (aero chamber & nebulizer). Contact
