JS scored twenty-eight out of thirty possible on the Mini-Mental State Examination. This shows no cognitive impairment without any clinical signs of cognitive impairment present. She received a perfect score on everything except two areas. The first one was only being able to recite only two of the three items to be remembered. Second area was difficulty counting back from one hundred by sevens (Demential Today website, n.d.). Mini-Mental Status Exam results show that JS is very alert, oriented, and cognitively aware. JS verbalized that her memory is not as good as it was and found that she needs to take notes or write information down to remember it. No other problems identified.

This score was not indicating depression. JS was very satisfied with her life and did not feel bored, empty, helpless, afraid, worthless, or hopeless. She was found to be in good spirits with the belief that she is better off than others, and her problems are the same as everyone else’s. The three that were bolded that indicated possible depression where having to drop many of her activities since she is not able to drive anymore, prefers staying home due to her limitations, and she does not feel full of energy. The apartment building is only one floor and has no stairs inside or outside. The doors are wide enough to allow her motorized chair and walker to easily pass. The bathroom has a high-rise toilet seat with grab bars. The shower has a shower seat for her with a grab bar. The bathroom has excellent lighting with space to maneuver with the walker (Tabloski, 2014). The youngest daughter is a nurse and keeps track of JS’s medications by throwing out the outdated or discontinued medications, refilling medications as needed at a local pharmacy, and filling a pill box biweekly. The daughter has written on each bottle in simple terms what each medication is for and the medications are kept in a hall closet.
