Because of the dysfunction of the eustachian tube the gas volume in the middle ear is trapped and parts of it are slowly taken by the surrounding tissues. Which then happens to negative pressure in the middle ear.
As its typical symptoms overlap with other conditions such as acute external otitis, clinical history alone is not sufficient to predict if acute otitis media are presented. This has to be completed by visualization of the tympanic membrane. Examiners use a pneumatic otoscope with a rubber bulb attached to assess the mobility of the tympanic membrane. The diagnosis may be made in children who have mild bulging of the eardrum and ear pain. A child crying can cause the eardrum to look inflamed to distension of the small blood vessel on it and mimicking the redness with otitis
