Ms. NS is a 21 year-old, heterosexual, Hispanic, single woman who is in her fourth year at the University of La Verne. Ms. NS’s major is Theater Arts and her minor is Business Management. Ms. NS presented to the Argosy University Therapeutic Assessment and Psychotherapy Services (AUTAPS) with her concerns that she has been unable to pass the math placement test. Ms. NS stated that the Disabled Student Services Department at the university referred her to the AUTAPS for her psychological assessment needs. Ms. NS is seeking a comprehensive assessment so as to receive accommodations, which Ms. NS is able to take an alternative course to fulfill the general education requirement. NS presented as a casual, slightly overweight female. She appeared indifferent and somewhat guarded as she kept her hands in the pockets during the initial session. Her speech was fast and downward inflection. Her vocabulary and grammar usage was indicative of her passion for reading. Ms. NS’s thought process seemed clear and linear. She did not display any noticeable psychotic symptoms during the interview, and her self-report also indicated that she had no history of psychotic episodes. NS reported that she has been having difficult time getting placed even in the lowest level of math class for about 3 to 4 years now. She stated that she never saw the seriousness of the problem until now that this is the last year for her at the University of La Verne. Ms. NS described about her usual course load that it had been anywhere from 18 credit hours to 22 credit hours. Furthermore, Ms. NS stated that she has other extra curricular activities for the theater arts program, and she is also a key member in the sorority group who disseminate information to all members and organize events for the sisters. However, Ms. NS informed that she had to reduce the course load to 12 credit hours this semester in order to focus on her senior project that is part of the graduation requirement. Ms. NS reported that she had never been diagnosed with any psychological disorder in the past. Although Ms. NS’s main concern is her inability to get placed in the correct math class, she exhibited some emotional distress during the intake. Similarly, Ms. NS’s responses from the intake packet and the Rotter Incomplete Sentence Blank corresponded her current emotional struggles.
