Social Work Case Summary

I am a Social Worker for the Providing Hope Mental Health Center in Plentyville, Nebraska. New clients are assigned to the next available social worker and today I was given the opportunity to meet a woman named Amy. Prior to our meeting, I was able to read a basic client information sheet, this is where I learned Amy had requested to speak with someone who specializes in domestic violence issues.
Identifying Information Amy is a 35-year old Caucasian female. She was born, raised, and continues to live in the small mid-western town of Plentyville, Nebraska. Amy is currently unemployed and living with her mother. She has never been married and does not have any children. When asked about her marital status, Amy mentioned an on again,

There are no indicators present for any of the personality disorders.
Related Medical Disorders. There are no known related medical disorders, although Amy was encouraged to see a physician to rule out the possibility of a medical condition.
Psychosocial Stressors. Amy’s psychosocial stressors include: the divorce of her parents and estranged relationship with her father, overprotection from her mother, estrangement from long-term boyfriend, unemployment, unsatisfactory housing or living situation, financial instability, emotional and physical abuse in her current relationship, and separation from a close friend.
Clinical Case Summary Amy is facing some challenges with the loss of her business and significant other. These losses have left her feeling unmotivated, worthless, and lost. Even though Amy has lost interest in being a baker, her past shows that she possesses vast culinary skills. Plus, she has a good support system with a mother and grandmother that are highly positive, encouraging and supportive. She recognizes her current boyfriend as being abusive; subsequently she would like to work on her self-esteem issues. In addition, she is at a point where she is ready to change her current behaviors to help deal with the depression, so that she can stop sleeping all the time, as well as, enjoy the things she once did, such as baking and spending time with family and
