Urine drug screen dated 05/06/16 showed positive findings for hydrocodone and hydromorphone.
Per the medical report dated 07/22/16, the patient presented for bilateral knee pain. Current medications include Lidoderm 5% patch; hydrocodone/acetaminophen 10/325mg; hydrochlorothiazide 25mg; omeprazole delayed release (DR) 20mg; Aleve 220mg; and Zanaflex 4mg.

She has completed physical therapy with improvements in pain, strength and range of motion (ROM). She is still having some weakness and pain in the right knee. Surgery has also been recommended for the left knee, but she is holding off on this for now. She continues with home exercise program to strengthen the right knee and to normalize her gait. She also utilizes Norco, which does help with pain and function. Norco helps reduce her pain from 8/10 to 0/10 on VAS. With medication, she is able to walk better, exercise better, better range of motion and can tolerate activities that require more strength. She is tolerating this medication well and denies any side effects. Of note, DEA cures was consistent. She does have updated signed pain contract. There is no sign of aberrant drug
