Medication Therapy Management (MTM) is a service that pharmacists provide to improve treatment outcomes for individual patients by optimizing the safe effective use of medications. The services that MTM provide incorporates the following: medication therapy review (MTR), personal medication record (PMR), medication-related action plan (MAP), intervention, documentation, and follow-up (4). These services are provided to Medicare patients at no cost and pharmacists who provided these services can be reimbursed by Medicare Part D.
