SC received VM from CG Mellissa on 1/1/2016 stating that she escorted Pa to a medical appt on 12/31/2015. SC placed monitoring call to Pa on 1/05/2016. SC asked Pa how she was feeling and she replied “very good”. Pa reported she had an outpatient same day surgery procedures called deveining to removed blood clot from her dialysis port. Pa reported no falls, new heath issue, ER/hospital visit or medication. Pa reported she is receiving services as it is outlined in Pa's ISP. SC asked to speak with the DCW's Melissa Hayes (Pa's dtr's) but she was not available at the time. Pa states that Melissa is still her primary caregiver and she assists Pa bathing, dressing, grooming and IADLs, meal preparation, and housekeeping, 7 days x 4 hrs. Informal
