Medical Emergency/Overdose
On 08/23/2016 at 1024 hours I was dispatched to 4148 102nd Pl n in reference to a possible overdose.
Before I arrived on the scene I was advised by dispatch FD was transporting the subject to Northside Hospital at that time. I arrived on scene and attempted to make contact with the complainant (Sheila Van Der Leeden) who was the subject’s wife. While on scene initially it appeared no one was home and all the doors had been secured after I walked the perimeter of the house.
I then relocated to Northside Hospital in an attempt to speak with the subject who overdosed. Upon my arrival on scene I made contact with medic D. Twardowski ( 35A squad Run# 6128077). Twardowski advised when he arrived on scene he was led to

Sheila has been staying at the La Quinta Inn since the domestic battery incident (16-58900) which took place on 08/03/2016.
Next to Gerared’s bed there was an empty pill bottle of Phenobarbital (last filled on 11/27/2015 with 180 pills) and a bottle of vodka which was mostly empty. Pictures of these items were taken and added to this report. I asked if Gerared had ever been suicidal and she stated no. Sheila further advised when Gerared has trouble falling to sleep, he will mix the pills with vodka. Sheila further stated he usually does this about once every three months. Sheila then stated Geraredhas not mixed the pills and vodka in a little over a year due to him going in to a coma for two days the last time he tried this.
I then attempted to make contact with Dr. Villamagna; Gerared’s primary care provider. Villamagna was not in the office but I was able to leave a message with his messaging service and I was advised he would get in contact with me when he returns on 08/24/2016. I will supplement this report when I can gather more information on Gerared’s condition from Dr. Villamagna. The last update I received on the condition of Gerared; he was still
