1. Medical Conditions/Diagnoses:
Pa has a diagnosed hx of cataracts in both eyes, is partially blind in her L eye and experiences blurry vision in her R eye, bilateral knee replacement and breast cancer. The Pa is currently being treated for the following DX: HTN, LE edema, OA in knees and shoulders, and hypothyroidism. Pa experiences dizziness, weakness, fatigue, pain in knees rated 8/10 before meds, LE swelling in legs, ankles and feet, stiffness in knees and shoulders, weight gain and unsteady gait. Pa had no recent hospitalized nevertheless she reported one fall while trying to stand up. Pa did seek any medical treatment. The Pa appears to be exhibiting signs of cognitive impairment.

She has chosen Dynamic Home Health Care as her provider of choice and would like her niece, Yolanda Ross, to be her PAS aide. She would like her niece to assist for her 2 hours Tuesday, Thursday and Saturday from 11am to 1pm. Pa is very unsteady on her feet and due to her LE edema and stiffness, she is a falls risk. Pa has ERS button through Lifeline Systems to increase her access to emergency medical services. Pa currently receives home delivered meals through PCA she continues receive 7 frozen home delivered meals weekly through the PCA Meal Distribution …show more content…
