Medical Case Study: Albuterol

SC completed home visit with Pa at his apt. The Pa was admitted at Temple from 4/29/2016-4/30/2016 for Exacerbation of CODP. The Pa was prescribed one new medication Levofloxacin 500 mg to treat lung infection and three other forms of albuterol that he already have listed on his medication list in SAMS. According to the discharged paper work the Pa was admitted to the hospital for Exacerbation of CODP due to medication unavailability. The SC asked the Pa if he’s having trouble getting his medications and he said no. The SC informed the Pa of what the d/c summaries states and point out the discrepancy in what he reported to SC during monitoring call and what is written on the paper work. The Pa uses Girard Pharmacy and they do home delivery and contact the physician on the Pa’s behalf when they need new scripts (SC have made priors calls to pharmacy and learned of this). The SC asked the Pa why didn’t he call his doctor or pharmacy if he was out of medication and he informed the SC that “I don’t know, I guess I wasn’t thinking”. The SC asked the Pa if he needs help managing his medication and he informed the SC that he would like that. The Pa gave SC a VNA pamphlet and states that he would like to get a visiting nurse from there to help him with his medication. The SC informed that Pa that a visiting nurse would be very good to educate him on how to safely use his meds and create a medication schedule that he could benefit from. The SC informed the Pa that he would need to first get a referral from his doctor to get the process started. The SC placed call to Pa’s doctor office got VM with the Dr. Migrid cell. The SC called Dr. Migrid on her cell and spoke with her. The SC brief her on the Pa’s condition and she stated that she too believe that the Pa could benefit from having a visiting nurse managing his
