ALLEGATION
R/s on 06/13/2016, Edna Boahn was admitted to ICU at McLeod Medical Center-Dillion for opiates intoxication. R/s Mrs. Boahn has COPD, history of alcohol abuse, diabetes. R/s ambulance transported Ms. Boahn and reported she showed altered mental status and weakness. R/s Mrs. Boahn uses a walker and cane at home. R/s reportedly Mrs. Boahn’s home has no air condition and she was found dehydrated. R/s reportedly there are several dogs in the home and the home smells of dog urine. R/s Mrs. Boahn’s husband in a nursing home in Bennettsville.

R/s Dr. Benjamin Mitchell is asking for DSS to do home evaluation for abuse and neglect. R/s Mrs. Boahn was discharged today, but wasn’t released because the unknown male (allegedly nephew) showed
