Mary's mother provided input regarding Mary's ISP during Mary's annual meeting which was held on 12/15/16 at Hermitage Enterprises. Mary's mother informed staff that she would like to be notified ahead of time in the event that Mary wishes to exercise her right to vote. Staff will notify Dr. Shepherd in the event that Mary wishes to vote in the future. Mary's mother also requested that Mary purchase a new area rug for her bedroom floor to replace her current rug.

Mary provided input on what she would like to include in her ISP for the new year during her annual meeting. She stated that she would like to incorporate using the computer into her learning outcome for the year. Mary also came to staff periodically with requests to shop for personal
