On 13 March 2017, PV2 Santiago you were not present for the mandatory squadron accountability formation held at 0850 at the squadron motor-pool. You also failed to notify me or your chain of command that you would not be present. I received a text message at 0827 from PV2 Johnson that he dropped you off at Bennett Clinic after PT formation because you were throwing up. At 0915, I contacted the clinic and was told you were not there or was seen that morning. I then to Darnell ER, EBH (Embedded Behavioral Health) your barracks room and around the barracks common area and you were not there. I finally found you at 1540 in front of PVT Johnson’s room hanging out with PV2 Johnson. You stated you were not feeling well and threw up on yourself and
