We have been using two different approaches to help determine the client’s condition. The first is a psychoanalytic approach to determine how Mr. Rivers’ unconscious mind plays a role in his behavior. The second is a humanistic assessment to which we have taken into consideration how the client feels about himself, his belief system, and his overall behavior. We have come to realize that the client was severely abused and neglected by his mother, Callie Rivers. The traumatic events that have occurred in the client’s life have caused him to dissociate from reality by creating multiply personalities. We have witnessed Mr. Rivers disconnect from reality while his multiple personalities take over. Mr. Malick has given us a journal that was found after Rivers committed the murders which had different hand writing, writing styles and points of view, all written by his different personalities. The evidence obtained along with the symptoms, the structured interviews, and criteria needed by DSM-IV have lead us to our diagnostic, which is that Malcolm Rivers has a psychological disorder known as Dissociative Identity …show more content…
Malick decided the best approach to treat Rivers’ condition would be to begin Psychotherapy three times a week for the next five years. Therapy for this disorder is long term because clients have experienced repeated trauma (Ringrose, J. L. 2011). Psychotherapy for clients with DID is different to therapy with most clients because in theory, you're dealing with several different clients within one person, in this case ten. Dissociative Identity Disorder is a result of severe trauma that happened to the client in childhood, in Malcolm’s case his mother was a prostitute who abused and neglected him, this causes a child's mind to "break" creating the multiple personalities. The trauma has not been fully realized by the client so the main focus of this type of therapy is working through the traumatic events. The sessions will be about an hour and a half long each so that the client can talk about and realize the trauma that he has went through and still have time to come back from the emotional discussion and feel grounded again before the end of the session. The purpose of Psychotherapy for this particular client is to work through the traumatic events, integrate all ten personalities into one, establish a feeling of safety, and to reduce the blackouts and headaches (Spring, C.
