Occupational functioning is impaired because she feels like she is not being creative (decreased effort) and people don’t respect her. Psychological functioning is seriously impaired because she does appear to be disturbed because she is constantly crying, depressed and has suicidal ideation.

Her only episode she describes was the one after a supposed rape (she was not sure if she was raped). She ended up in the hospital for 6 days. This episode is not attributed to her medical condition, hypothyroidism. Her risk factors can be found in psychosocial, mental and environmental factors. Her mental factors include suicide ideation (she already have a plan), she is depressed all the time, she has a chronic health problem (hypothyroidism). Her environmental factor includes access to lethal drugs (Ambien), stress over losing her medical insurance which can prevent her from accessing mental health care, stress over money (going from 2 income to 1 income), and her family does not discuss her mental health (lack of family support). Her psychosocial stressor includes bereavement over her divorce; she feels people do not respect her and she also feels rejected.
