O: I want to learn better ways to cope with my depression.
A: MHS reinforces coping techniques taught in previous sessions.. MHS discussed implementing skills into the member’s daily routine.
R: MHS noticed the member’s mood to be very down (in what seems to be depression). The member openly expressed her feeling and concerns with MHS about her current mood. The member informed MHS that she has been feeling really down because of the lack of communication between her and her children. “I have not seen my children in over a month,” states the member.
