I agree, that one of the main factors of the two counties low ranks were due to large rural areas. They lack the accessibility of transportation to the nearest health care entity, jobs and food. Consequently, in an emergency the EMT may need to travel 25 miles before preforming life saving techniques. The lack of transportation hinders families in seeking employment, which increase their depression, drugs and alcohol factors. The government insurance (Medicaid) have distances restrictions and recipients are not able to get their appointments. I believe a few factors have a negative chain reaction, that increases other factors and poor quality of life.
