DOI: 3/11/2015. The patient is a 52-year-old female crane operator who sustained a work-related injury to her neck when she was hit by another crane while sitting in her scrap crane. As per OMNI, she was initially diagnosed with strain to multiple body parts.
Per the medical report dated 06/10/16, the patient complained of low back pain rated 6/10 with distal lower extremity tingling sensations. She was previously sent to physical therapy and had 60-75% improvement of her symptoms. She has not had a pain management consult. She experiences a tingling sensation from the ankles that radiates upward into the calves. The symptoms are worse while at work. She further reports having leg weakness during transitions at times.
