Love is a 19-year-old Black mother to a baby boy named Donyaeh. Love is presently dating a Black man named Courtney but he is not Donyaeh’s father. With regards to Love’s appearance she dresses well in public, and expresses herself well; she is quick at correcting those who misunderstand what she is expressing. Love is able to articulate her feelings properly, but most of her answers or sentences begin with “I don’t know”. Love is struggling with depression and anger and as a result of it she becomes irritated quickly and starts to talk loudly to the point where she breaks out in rages. From my observations, it seems that Donyaeh’s speech and mobility are appropriate for his age. The medical professional that saw Donyaeh mentioned he had increased in weight greatly from his initial weight at birth. The client has not been able to cope with stresses relating to stressful events. In regards to the family composition and history, client is one of Diane’s six children. Charles the eldest is deceased, followed by Tameka then Trenise17, Morean 15 and lastly, Willie 14 who happens to be struggling in school, and is presently living in a group home. All of the client’s siblings were in foster care system for six years; but have been placed back with their mother within the last four years. Diane controls the home, while Tameka makes decisions since she acted like the mother to her siblings when Diane was on drugs. The client feels that her siblings make fun of her and as a result, she cannot have a functional relationship with them. All of the members of this family have dealt with so much that getting along is not an option. As for Willie he preferred living in the group home than to be returned to his families home. Previously client expressed having a need to prostitute herself to make ends meet. Client contracted HIV at a previous point in her life. Please see client’s intervention plan for the goals and objectives taken to achieve goals. Some of the referrals to other agencies include seeking supportive counseling, educational workshops, peer support groups, parent-child playgroup from Queens South Youth Parents Program @Catholic Charities at least once a week until child neglect case is removed from her file. Another referral will be Beth Israel Medical Center, an agency for adult consumers with depression and bipolar disorder as well as a concurrent group for their friends and family. Client and her family should attend 2 times a month for 1 year. The factors that may affect the goal achievement may involve client-losing motivation, also client’s family not participating in the helping
