Lorazepam In The Bottle

Anonymous staff thinks that resident Nola Gucliano did not die of natural cause; RP believes that her death was due to the improper care and medications being administered to her without orders. Around 8/15/17, RP noticed that staff signatures are being forged on the controlled substance page and the entire page was "redone". RP stated that medication bottles are being stripped of their labels and the medication is being given to other residents who do not have the medication prescribed to them. RP that a resident was given Lorazepam without orders and another resident was out of Warfain and was given another resident's Warfain. RP stated that the medication bottles will be labeled "multivitamin" but will have Lorazepam in the bottle instead.
