Long term conditions are a major challenge in a health care system, but there is a systematic approach that can provide the client for self-management and help prevent future complications. This requires a strong leadership, and lots of team collaborations. Preparations is important because we can’t make a lifelong decision without thinking through it, so it is important for all the patient’s providers to gather information such as test results, and create question to how to prepare before making decisions and this includes a great participation of providers, nurses and others such as family members and patient.
