History/Subjective Data:
S.V. is 54 years old white Caucasian male, with Bosnian ethnicity, having Bosnian as his first language, married for 28 years to his wife, having Islam as his religious background without practicing it, and having deep belief in helping others in need. He believes that if he helps others in need, God will grant him the good health. He denied having any religious or cultural beliefs that might affect his healthcare. He loves gardening and fixing garden machines what help him to relieve the stress. The client has only a bee sting allergy (carries Epi-Pen); takes no medication on a regular basis (occasionally takes Tylenol for minor aches).

The step length was about 15 inches from heel to heel. Further, the client observed staying balanced when walking a straight line in a heel-to-toe fashion (tandem walking). Also, he was able to walk on his toes, then on his heels for a few steps, without any difficulties.
The Romberg Test: The client was able to maintain stable position for longer than 20 seconds, when asked to stand up with his feet together, with his arms at the sides, and with his eyes closed. Then, the client was able to perform hop in place with one then with other leg without any difficulties.
The client was able to perform rapid alternating movements (RAM), as he was patting knees with both hands, lifted them up, turned hands over, and pat the knee with backs of the hands, then doing it with equal turning, and with quick, and rhythmic pace. Furthermore, the client was able to perform finger-to-finger test with smooth and accurate movements. Further, the client was able perform finger-to-nose test with accurate and smooth movement. Then, he performed heel-to –shin test moving his heel in a straight line down the shin,

When triceps tendon was struck directly, just above the elbow, +2 response observed, with extension of the forearm. Further, branchoradialis reflex was tested, when forearm was struck directly, about 2-3 cm above the styloid process, and +2 response with flexion and supination of the forearm observed. When quadriceps reflex (knee jerk) tested, it was struck directly, just below the patella, +2 response observed with extension of the lower leg, and palpation of contraction of the quadriceps.
Achilles Reflex (ankle jerk) was tested by directly striking Achilles tendon, and holding the foot in dorsiflexion: with response +2 and plantar flexion of the foot against my hand, bilaterally. Further, no clonus observed, when stretched the muscle of the lower leg and briskly dorsiflexed the foot, bilaterally.
Superficial (cutaneous) reflexes:
Abdominal reflexes: Observed ipsilateral contraction of the abdominal muscles with the deviation of the umbilicus toward the stroke, when the handle of the refax hammer was moved from each corner of the abdomen toward the midline at upper and lower abdominal
