Family: Courtney is a 17 y/o Caucasian female who was born in Scotland. Courtney is an undocumented immigrant. She continues to live in Clark with her Nikki Matear, mother, Walter Poszyler, mother’s fiancé, her 16 y/o sister. At this time Ms. Nikki reports that Courtney has been doing well. She reports that Courtney best friend recently passed a way from a drug overdose. Nikki states that that she feels that Courtney best friend passing away was an eye opener for her. Nikki reports that Courtney has also broke up with her girlfriend which she has been taking well.

Psych/Mental Health: Courtney diagnoses are F32.1-Major Depressive Disorder, single episode, moderate. She was taking Wellbutrin 200 mg in the past. At this time Courtney not on any medication. Courtney also is not receiving therapeutic services. CM provided the family with information to Proceed. Proceed has a grant funded program that will provide counseling to undocumented youth. Nikki feels that Proceed agency to address Courtney emotional and behavioral needs.
Last month CM provided Ms. Nikki information to Proceed for counseling services. At that time Ms. Nikki was not open to sending Courtney to Proceed because her concerns of negatively peer influence. At this time Ms. Nikki is willing to pay an affordable sliding scale fee for Courtney to attend weekly therapeutic services. Nikki would like for Courtney to attend CCS but due to her finical situation she is not able to afford the weekly $150.00 sliding scale fee. CM will talk to CCS intake coordinator to see if she is able to work out a reasonable weekly fee out for the
