LEARN Model for Refection
L– Look back. Play back every detail of the experience in your head like a video. It was my 1st day at the clinical. We were posted in 5B medical unit.. The main aim of our 1st day clinical was to get an idea about the routine in the unit, how the nurses are providing care, getting an idea about using electronic charting. I was partnered up with one of my classmate and we both were assigned to do shadow a registered nurse from the unit. It started with shift report, and then we went to each of the assigned patients and introduced us. After that I assisted my nurse in taking vital signs, and I got the opportunity to do vitals. Then my nurse started doing medication. It was totally a new learning for me. One of my patient who was admitted with diabetic ketoacidosis, and over a period of few days he recovered from ketoacidosis. On that day morning his blood sugar was 8.8mmol/L, and his medication order is to administer Insulin subcutaneously according to sliding scale. The nurse prepared the medication and went to patient’s room. The nurse explained the patient that his morning blood sugar is little high, he need Insulin to bring it back to normal level. The patient was not ready to take insulin. He was saying he don’t have diabetes and there is no need of taking insulin and he wants to have a talk with his son regarding this matter. The nurse tried to educate him regarding diabetes and the importance of keeping blood sugar normal, but still the patient was not ready to take the insulin. The nurse didn’t administer the insulin and she recorded everything and left a note for the physician too. The nurse respected the right of the patient to refuse the medication and tried her maximum to educate the patient. It was totally a new experience for me. I got an idea about how to deal with this type of situations.
