The problem is to ensure that elderly medical patients take their medication and recommended dose at the proper time to ensure that they receive the necessary treatment, and prevent the condition from worsening in the process. To solve this problem, a device will need to be implemented that allows the user to be reminded when to take the medicine, and also be sure that the patient is taking the correct amount of medication. Our first idea was to develop a portable watch device with built in audio and visual cues to remind the patient of their need to take the medication. The device could be programmed ahead of time by a technician, and instruct the user to take the recommended dosage at the correct time.…

Medical Assistants are responsible for scheduling a patient for appointment. A patient may call to schedule an appointment, so the medical assistant must collect information including name, address, date of birth, home and cell numbers. Sanderson (2017) states, “When a patient phones the offices for an appointment, the appointment can be scheduled in Office Hours before the patient information is entered in Medisoft. In these instances, it is possible to enter an appointment before a chart number has been assigned to the patient, by entering the patient’s name in the field right of the chart field” (p. 102). The patient must be given information on the medications they should be taking when the patient is discharged from the hospital. The Rosa Parks Wellness Institute for Senior Health (RP-WISH) created a program that focused on improving the safety of care by making an increased effort to schedule follow-up appointments and medication reconciliation within 1 week of discharge. They wanted to make this program because the RP-WISH office manager wanted clinical pharmacy specialists and inpatient pharmacists to be directly involved with medication reconciliation which they were never part of before. The plan was to help in care transitions in regards to medications by reconciling patients’ home medication use with primary care and hospital records (Liu & Garwood, 2015). They intended to call patients that were discharged home when the pharmacy finds medication-related problems and to intervene to resolve those problems to avoid adverse drug effects.…
