Kalahasthy's Office Case Summary

Dr. Kalahasthy’s office called about a CMN that was sent over and said that it states the patient is deceased. They called the patient and she was very upset that we had sent this to them. I think maybe someone put “d/c’ed” on the fax cover sheet and they took that as “deceased” but I’m not sure. I don’t really know what to do to rectify this situation. The patient called and spoke with PAC team who told her they don’t show anything in Brightree. I looked through everything myself and couldn’t find anything. But I just wanted to let you know because we don’t need any more CMN’s going to their office that say patient is deceased. The doctor’s office was very angry and the medical assistant I talked to said the patient was very angry
