Kaci Adams is a 13-year-old Caucasian female who lives with her parents and two brothers. She presents for treatment following a referral from her speech therapist. Kaci started to stutter when she was three years old. Kaci’s teacher expressed concern to the speech therapist during an in-school session. The teacher states that Kaci has become more easily frustrated when trying to speak during class. The teacher states that Kaci is becoming more aggressive with the other children in the class.
Assessment
Kaci presents slumped, quietly in a chair in the corner of the office. Her blonde hair is oily and pulled back in a ponytail. Her jeans are torn and her t-shirt is stained. She is tall and thin. She speaks very low. She is appropriately

Additionally, there are no longer any requirements for the use of speech interjections, such as “you know”, or “um”, which are also normally used for others without this disorder. Furthermore, there is the addition of anxiety and avoidance to the criteria for diagnosis, as this has been noted to be a big problem for many (Cohen, 2014). Kaci exhibits all of the aforementioned symptoms for a diagnosis of Childhood Fluency Disorder. Kaci is currently prescribed Xanax for anxiety.
Treatment Plan
Current Medication
Client is currently prescribed Xanax 0.5 mg BID for anxiety secondary to stuttering. Client has had no adverse reactions to current medication. Client continues to have outbursts and uncontrolled behavior at current dose. Social Worker will refer to prescribing doctor for possible dosage adjustment or medication change. Medication is administered by parents daily and client is compliant with taking medication. She is not prescribed any other medications. Social Worker will continue to monitor medication compliance. Client is not suicidal and has no plan to hurt others.
Immediate Goals
Client will begin counseling with Speech Language Pathologist to help to improve her quality of life (AHSA 2017) as evidence by a reduction in inappropriate

Social Worker will attempt to decrease the clients level of anxiety with treatment. The Severity Measure for Generalized Anxiety Disorder tool and the Kutcher Generalized Social Disorder Scale for Adolescents was utilized.
What does it measure? The Severity Measure for Generalized Anxiety Disorder measures the severity of generalized anxiety disorder in children and adolescents aged 11-17 (Andrews, Bogels., Craske, Lebeu, Stein, & Wittchen 2013). The tool should be completed the child upon receiving a diagnosis of an anxiety disorder and thereafter, prior to visits with the clinician (Andrews, Bogels., Craske, Lebeu, Stein, & Wittchen 2013). Each item asks the child to rate on a 5-point scale the severity of his anxiety during the past 7 days (Andrews, Bogels., Craske, Lebeu, Stein, & Wittchen 2013).
The Kutcher Generalized Social Anxiety Scale for Adolescents is a clinician-rated scale designed to assess the severity of social phobia in adolescents (Brooks & Kutcher 2004).
