Step 3. Evaluation: During this course evaluators visits the family and the child to determine eligibility, gather information from what they observed from that child, and submit both summaries and reports to their supervisor. If the child is eligible, than an Individualized Family Service Plan meeting is scheduled, which would be step 4. Family would then deliberate what their expectations are as far as the outcome for their child, and proceed to agree with a family service plan. An ongoing service coordinator will remain with this family to answer or assist them with anything that they are in need of. Step 5.
