Joseph Smith is a nine year old boy. He has recently passed the third grade and will be starting fourth grade in September at P.S. 55. Joseph lives at home with his mother and two older brothers. His brother Michael is 15 years old and attends Tottenville High School. Matthew the middle child is 11 years old and attends Intermediate School 34. Joseph’s father, John, lives in New Jersey with his girlfriend. Joseph’s father visits him and his siblings every other week. Joseph doesn’t speak of his father much and is a shy boy. 55 since Pre- Kindergarten. He is known to be quiet in class but has a good relationship with teachers and peers. In first grade, Joseph was diagnosed with a minor language disorder. He has difficulty expressing his needs, information or understanding what others say. As a result, he has received speech twice a week in a group for a half hour. The speech therapy services have seemed to help him but he still needs extra assistance for comprehension.
Previous Referrals/Consultations At an early age Joseph had no sign of a language disorder. He can speak fluently and answer questions. Joseph’s disorder was first noticed by his first grade teacher at P.S. 55. His teacher realized he can read at a moderate pace, but was not grasping the concept. Joseph can recognize words but has difficulty retelling stories; prediction; stating the main idea and inference skills. His teacher contacted his mother of his difficulty and suggested he be further analyzed. Joseph’s mother took him to specialist and was diagnosed with language disorder. He did not need any other special services other than his speech therapy in school. Joseph can read fluently at level “O”. His comprehension skills have deteriorated in third grade and his mother is concerned. Mary has contacted me at the beginning of summer to assist Joseph as a reading tutor focusing on

Joseph was quiet at first, but once I could relate to some of his favorite things he opened up more. I felt Joseph would benefit from more social interactions as well. He keeps to himself, his brothers don’t seem to include him in much and he has a disability. Joseph was very cooperative and had a positive attitude during testing. He seemed to enjoy the lower grade work but became somewhat frustrated when we reached grade level. I let Joseph know if he should become agitated, he should use his words and ask to take a five minute break. After taking a break, Joseph was at ease and could continue with his work. I used a rewards system to encourage Joseph to do his best. He would receive a sticker each day he completed his work and was allowed to pick something from the prize box after receiving five
