History Jon mother brought him to this clinic. According to the written case history supplied by his mother, Jon is the middle child in a family of three children. At 24 months, Dr. Miranda Lambert, Jon’s pediatrician evaluated Jon for tympanostomy placement due to his frequent ear infection. At 24 months, Dr. Mark Sloan, an ENT, inserted tympanostomy (PE) tubes. Jon experienced colds and draining ear at age 2. At 18 months, Jon suffered with a high fever. Dr. Carrie James, an audiologist, evaluated Jon hearing a year ago and concluded Jon hearing is normal. With respect to early speech and language development, Jon reportedly spoke his first words at 1 years and combined words at 1.5 years. Mrs. Langston reported that Jon never has engaged in conversation. Jon communicates using “short phrases and gestures”. Jon mother revealed her son never uses any words or commands and will moan or grunt. Jon “seems lost” when instruction go beyond one step. According to the case history, Jon’s father had a mild speech problem as a child and Jon’s sister received speech therapy due to a lisp and “not talking much”.
Jon currently attends Happy Day’s Daycare and is in the 3’s classroom. His mother reported that he is “behind others” academically.
