The identity of the patient is secured by using the initials “JD” instead of using her full name. JD is a 67 year old cisgender female, born on January ninth, 1951. Her ethnicity is Caucasian. Catholic is her religious preference. JD identifies as heterosexual and is married. In conversation she stated she had been married 4 to 5 times. JD also stated that she was once a student at the University of Portland where she studied nursing. Her highest level of education is then some college. She stated that she did not work before coming to OSH, and her sister helped her pay for things. She said that before coming to Oregon State Hospital (OSH) she lived in an apartment by herself, and that when she gets out she would like to live with her sister, husband, or a group home. …show more content…
JD has been diagnosed with schizoaffective disorder bipolar type. Additionally, she is also diagnosed with chronic kidney disease, unspecified feeding or eating disorder, insomnia, mild neurocognitive disorder due to multiple etiologies, depression, suicidal ideation, tachycardia, and psychological factors affecting other medical conditions: not eating- malnutrition, sepsis, renal failure, dehydration, and constant walking- foot ulcer. This is JD’s 2nd psychiatric hospitalization at OSH, but she has been hospitalized for psychiatric reasons 10 times throughout her life. She has been admitted for attempted suicide. She has tried to commit suicide at least three times throughout her life. Once by cutting her wrists, once by walking into the ocean, and once by undressing and running out in front of cars. She was at Saint Vincent Hospital for six months before coming to OSH three months
