Jacob Observation Paper

Session held at the SVABA center. When Jacob arrived to session base on observation he seems little bit tired. DI ask the bus driver if he was asleep in the bus she said no he was awake all the time. DI look at Jacob eyes there were irritated but Jacob mentions that his eye hurt. DI took Jacob to the bathroom for potty time, Jacob was able to urinate in the toilet he only need it assistance to put his pull-ups on. DI give Jacob snacks and then took his to the quiet room to rest. Jacob did a great Jacob responding to general knowledge questions.
