1. Please describe the setting in which you supervised the applicant. Give details about their responsibilities and relationship to patient care.

I am a wellness nurse in a memory care dedicated living assisted living facility. I have known Jache since she came to work at Sunrise on September of 2016. In our facility, Jache is a designated care manager for 7 of our residents. As a care manager, Jache assists residents with various activities of daily living such as perineal care, assistance with eating, dressing, showering, grooming, and any scheduled social activities. Since Jache mostly works for us during the evening shift, she also assists our residents to bed. Though Jache is new to our facility

There was a time during which another care manager was required to measure the temperature of a resident who had recently fallen, but the resident was displaying aggressive behavior in which prevented the care manager from performing the task. Jache was able to notice that the resident felt uncomfortable with the temporal thermometer. At first, Jache suggested the use of oral thermometers however, I informed her that oral thermometers were currently inaccessible. Jache was able to calm the resident by demonstrating to the resident the use of the temporal thermometer on herself and explained that it was only going to be in her ear for a short moment. Jache was able to measure the resident’s temperature by counting down outloud to ensure the resident’s comfortability.

At first it was difficult for Jache to have emotional stability due to having little to no amount of health care experience. To suddenly care for a declining patient makes it understandable for a person of Jache’s experience and age to easily display dejection. Especially for someone like Jache who is filled with compassion.
