IRTC observed documentation entered in Connections on 5/8/17 by CPS, Nichole Stuart, which depicts that CPS observed the subject child on 5/3/17. Subject child was observed with an injury to her ear which no one except subject child, Sophia, was able to provide an explanation; Sophia informed CPS that Xavier “fell and hit his face.” CPS is to assess the validity of Sophia statement and obtain any medical information regarding the injury. If it is deemed that it was inflicted by another person (adult or child) and it was a serious injury, CPS is to contact IRTC or an IRTC at ECS (after hours) for a case reassessment.
