On 06/23/2016 at approximately 1728 hours, I was notified by Hillsborough County Sheriff 's Office dispatch of a traffic fatality on County Road 579, south of Hillsborough Street. The crash involved a Toyota Tacoma and a single occupant. I responded to the scene with Detective M. Carter #181968 and Corporal J. Graves #4044. Upon arrival, I observed the involved green Toyota Tacoma at final rest in the ditch on the east shoulder of the roadway. I was assigned as the lead detective and Detective M. Carter was assigned as the scene detective. Deputy S. Gray #3277 was on scene prior to my arrival and was assigned the DHSMV crash report #86559690.

Carter that he completed an interview with the driver 's son, Jorge Lopez, whom arrived on scene after the crash occurred. I was then informed, the witness that performed CPR on the driver was on scene. I completed a recorded interview with the witness, Jose Garcia. He advised he did not see the crash occur, but stopped to render aid and began CPR when he observed the driver laying on the ground next to the passenger door. He explained that he observed the driver laying on his right side upon his arrival and rolled the driver onto his back to begin CPR while his worker called 911.
I walked the scene a second time and observed multiple Modelo and Coors Light beer cans in the truck bed and in the ditch behind the truck. I also observed the driver seat belt in the retracted position.

I observed the exterior of the truck to be covered in a layer of mud from the impact with the water filled ditch. The passenger side of the truck had dents from the impact with the embankment of the ditch. I observed the rear passenger side brake light lens to be broken, as well as, the metal housing surrounding the light. I observed the tailgate to be distorted from the impact, causing the driver side brake light lens to break also. I observed mud wedged in the passenger side headlight housing and grassy wedged between the rim and the tire from the vehicle 's rotation and sliding through the grass. I observed the interior of the truck to be heavily worn and the driver seat belt to be in the retracted position, but not locked in place. Not airbag were deployed during the crash and the vehicle was not supported by the Bosch Data Retrieval system. I observed spattered mud on the interior of the truck cab, concentrated on the passenger side. I observed an empty Bud Light beer can on the front passenger floor
